Community Support Network

The Community Support Network partners with local organizations to staff community health workers who work in the community to provide support to residents by having meaningful conversations about daily life challenges and possible strategies, solutions and referrals to service providers who can help with:

  • Daily concerns related to physical health
  • Concerns about safety at home and/or in the community
  • Strategies to deal with stress, anxiety and emotional health
  • Connections to food and housing
  • Connections to education, career and job-readiness
  • Connections to substance use supports and treatment

Everyone experiences daily struggles that cause stress. Daily stress builds over time and may become tough to handle alone.
